AICM Field Trip to Miracle Strip Amusement Park

Assalam Alaykum Parents,

We are planning a field trip for all students from AICM on 31st May 2015 (Sunday).

Ø  Please dress the kids modestly in GREEN long T-shirt.

Ø  Please drop your child at sharp 7:15AM @ AICM with spare clothes and lunch box.

Ø  We are planning to be back  Inshallah by 8:30PM

Ø  Please pay $35 towards the field trip expense for each kid with this signed form now.

Ø  If you want to join, you have to provide your own travel & ticket and you can Chaperon your kids.

I am the parent/Guardian of _________________________________, I give permission to AICM Sunday School to take my child to Miracle Strip Amusement Park and I give permission to give emergency  medical treatment to my child in case of a medical emergency.
